New Patient Form
If you're a first-time patient, we recommend completing our new patient form early if you can in order to save some time. If you don't have the chance, please arrive at least 10 minutes early for your appointment to complete the form with our staff.
Your dentist will start by reviewing your dental and medical history and completing a comprehensive dental exam. This exam will include a thorough examination of your gums, teeth, jaw and bite, an oral cancer screening and any required X-Rays.
Based on your examination and medical/dental history, your dentist will create a long-term dental treatment plan for you. Your dentist will also dedicate time to address any questions you may have about your dental health and treatment options.

To book an appointment at Monarch Dental, please contact our office during our regular business hours.
- How do I cancel or reschedule an appointment?
To cancel or reschedule an appointment, please call us. Please notify us about any change of plans as soon as you can, so another patient may be able to use the space in our schedule.
- Do you offer appointment reminders?
In the days before your appointment, you will receive automated phone, text or email appointment reminders from us. If you need to cancel or reschedule an appointment, please call us as soon as possible to let us know.
- Do you provide services in any languages other than English?
Yes, our office is able to provide dental services in several languages including Arabic, Italian and Portuguese.
If you have a language preference, please advise us when booking your appointment.
- Does your clinic have on-site parking?
Yes, we have free on-site parking available for all our patients.
Insurance & Financing
At Monarch Dental, we'll help you navigate our options for payment and direct billing to insurance companies, so you can focus on what's most important: getting the dental care you need.
- Direct Billing to Insurance
We offer direct billing for most insurance plans. For your convenience, we're able to submit your insurance claim electronically, resulting in faster reimbursement, without the need for paper forms and signatures.
Our administrative team can assist you in understanding your insurance plan better and can also send estimates to your insurance company prior to starting any major treatments.
Please bring your insurance plan information to your appointment.
- Payment Methods
You are responsible for any co-pays or amounts not covered by insurance. These must be paid at the time of your appointment. We accept payment by cash, debit or most major credit cards.
We accept payment by cheque, debit or most major credit cards. For the time being, we are not accepting cash per our COVID protocols.
